Explaining the 45-year decline in so-called 'Mainline" Protestant churches can't be reduced to a few bullet points. The decline stems from complex sea-changes in American religious culture. 

The way forward is straightforward enough: get beyond Sunday worship as the main locus of ministry, and deploy multiple ministries -- on-site, off-site and on-line -- to reach a diverse marketplace. 

But Sunday worship is so ingrained in mainline consciousness -- it's what we do, it's who we are -- that it takes serious work just to open the door to fresh ways.
